- Open the link. It will take you to the questions or information that still need to be completed.
- Answer every required question.
- Submit your answers for review.
Sometimes the clinical team may ask for more information after reviewing your first answers. Use the secure link you receive to provide it.

If you already submitted the form, it may still be processing or waiting for the clinical team to review it.
